Facilities

We have an in office AAAASF certified operating room which is one of the largest outside a hospital. For our patients' safety, we far surpass the minimal requirements of the certifying agency. Anesthesia services are provided by certified nurse anesthetists. I've been working with the same group for over 16 years. All our major cases are done under intravenous conscious sedation and /or epidural anesthesia.

Dear Michelle, Labia minora reduction or labiaplasty is a very common procedure in my practice. It is the 4th most common surgery I do. There are different methods of reshaping the labia. I find that often the excess clitoral hooding needs to be addressed at the same time. I do these surgeries in our certified office surgery suite under local anesthesia with minimal oral sedation. See a board certified plastic surgeon or gynecologist with experience in female genital cosmetic surgery for an in person consultation/evaluation. Remember, not all are experts at these delicate surgeries and if the outcome is suboptimal, it might not be possible to create a normal natural result later. Good luck.

Poor outcome?

Sorry for your poor surgery. I personally never see an incision placed in an obvious visible place on the torso. I personally, place a one cm stab wound within the areola ( the pigmented skin around the nipple) and one in the armpit. This leaves no visible scar, no sign that surgery was ever done. Most surgeons place a half circle incision around the lower pole of the areola, I believe this is not necessary either. I remove excel breast tissue through the 1 cm incision. What’s more upsetting, that your results are not good and you also have a long visible scar. You still have gynecomastia and poor definition of the male chest. You need to find your surgeon or raise your concerns with the hospital. A surgeon cannot just disappear. Good luck with your healing. George Marosan, M.D.

Thank you for your question. Based on your photo that you posted, you are a good candidate for body sculpting and gynecomastia surgery. Not all your requested procedures can be done the same day. Although your gynecomastia is very large and might need skin excision, I prefer to treat this with VASER liposuction first. At the same time, your abdomen can be sculpted, including the anterior and lateral chest wall, whole back, love handles. This would give you the best body contouring. The facial surgery can be done separately. Would need better photos frontal and lateral views to make meaningful comments about the face. I do my VASER Hi Def Lipo via stealth incisions, with no sign that you had surgery. If you need skin excision later for the chest, this will leave a scar. Thank you for your question. Insurance companies have certain criteria that need to be met to cover the surgery. Find out what your insurance company’s criteria is. I, personally, stopped dealing with insurance companies. I prefer to make my own decisions and provide what’s best for my patients and not what an artificial criteria requires. I prefer to do a short scar breast reduction/lift and not an anchor scar. I do these surgeries in our certified office surgery suite under intravenous sedation only. If your daughter desires surgery, should wait until her spring or summer break from school. Thank you for your question. First of all if you are scared, you should hold off with surgery. Go to a few more consultations with board certified plastic surgeons and seek other opinions. Without photos, it is difficult to give specific advise. It sounds that your surgeon proposed a tummy tuck and a breast reduction/lift at the same time. A standard tummy tuck is a two dimensional operation. In a modern tummy tuck , I personally approach the body in three dimensions. This means , that to get an hourglass figure, the back, waist and hips need to be treated with liposuction 360 degrees and at times fat is transferred mainly to the buttocks for shaping and enhancement if that will give you an athletic feminine silhouette. I have patients who got excellent contour improvement from VASER Hi Def Lipo without a tummy tuck after. Revision surgery will cost you more than canceling surgery!

Saline implants can be deflated first and removed through a 2cm incision in the breast fold. Tissues are thin here and one does not need to dissect through the breast tissue if the areolar incision is used. Good luck.

Full or mini TT or other options?

Based on these photos there's no minimal invasive procedure that will give you a satisfactory result. You need a full tummy tuck, most likely skin only and VASER sculpting of your 6 pack and gynecomastia surgery via intra-areolar incisions and Renuvion skin tightening of the chest. Not being able to evaluate your back, this is the minimum approach for a sculpted result, but much improved from your posted 4th photo. Thank you for your question. Without photos, height and weight it is difficult to provide specific answers. To get the best body sculpting results, I use VASER Hi Def Lipo 360 in a patient who has good elastic skin, not loose, thin skin with stretch marks. Abdominal etching in females is different than in a males. When I do a BBL in conjunction with the lipo 360, I want my patients to sleep on their abdomen or stomach for 4-6 weeks minimum. This would create a problem with a breast augmentation done at the same time. Personally, I do not recommend the breast procedure with the body sculpting procedure together. you can get your augmentation done 6 weeks post body surgery. I do all these procedures under deep intravenous sedation and not general anesthesia. I'm sure you can find someone who will say that you can do these in one operation. Yes, I can do it too, but is this prudent? See a board certified plastic surgeon for an in person consultation/evaluation. Good luck.

Thank you for your question and photos. It looks like your TT incision will heal well. Trying to “reattach “ the BB 4x in weeks is a mistake. Leave it alone to granulate in. Right now it is very large, unnaturally round. Let it heal in by itself. Might get significant scar contracture, and will look more natural. 4-6 months later might need a revision or might be just fine. Trying to fix it weekly, will cause more harm, wider wound when you try to suture very inflamed tissues. I would not run for the 5th repair in that many weeks. It will not work. Sometimes the hardest thing is to do nothing, being pressured to fix something before the tissues are “workable” again, takes time. Good luck with your healing.

Thank you for your question. To get the best body contouring result after massive weight loss, you need to consult a board certified plastic surgeon experienced in advanced body sculpting techniques. Although you are are focused on the abdominal loose skin, there’s loose skin all the way around. I’m doing a similar surgery on a young man next week for the same reason. The chest might need to addressed at a different time after the body surgery. There are different techniques to minimize the scars and also place them in a location to heal well and be minimally noticeable.
