Your initial consultation will take about 1.5 to 2 hours. You can valet park in the garage at our building.
Our front office concierge team will sign you in and ask you to complete paperwork. You will then meet with Dr. Cassileth so you can get to know each other, discuss your plastic surgery options, and get answers to all of your questions. Dr. Cassileth will examine the areas that you are interested in improving, and together you can determine the best options to accomplish your desired goals.
Our staff will then take photographs for you and Dr. Cassileth to use in planning your surgery and evaluating the results. Your Patient Care Coordinator will meet with you to discuss costs, aftercare, and answer your questions. You will be able to take home a detailed cost estimate for your procedure on the same day.

Breast reconstruction, gel versus saline

Ask your PS to feel saline and the silicone implants, they should have samples at the office. There is also a newer saline implant on the market, called the "ideal"implant, that is far less likely to ripple than traditional saline, but a little firmer than silicone. I let my patients feel the different implants, and most decide that the cohesive gels are safest and most realistic. There is a rare breast cancer called ""BIA-ALCL" that appears to be associated with textured implants, so although unlikely, most patients are choosing the smooth round to avoid the risk. When you swap the expander, ask your surgeon to perform fat grafting. I perform typically 100cc of fat grafting per side, which make s the breasts softer, hides the implant, and warmer! Make sure your surgeon is very comfortable with grafting, as I've found some surgeons perform this more than others.

Fat transfer to the breast is a wonderful procedure, done right! First, the liposuction has to be, of course, amazing. If you have some areas that are hard to lose that fat from , those are perfect, as using that area will allow your breasts to keep their size even with weight loss. I personally love arms and knees, which look great with every bit of fat removed. Next, the grafting must be meticulous,, with the fat suctioned with low suction strength, all oil and tumescent removed, and packaged into small syringes. I use 3cc. THEN, the fat is injected in 0.1 - .3cc passes, with many pinpont access sites. It may take 1,000 passes to correctly place 100cc of breast tissue, but this avoids fat necrosis, which you get when lumps of fat are injected. FINALLY, don't overgraft. A good rule of thumb is you can double your breast size, so make sure that this is what you want. If you are an A and want 400cc implants, it will not be enough. But if you had kids and lost a cup size, perfect!
