Are you serious? Are you planning to put your face into a “pseudo Certificate” of a specialist trained specifically to give care to the peoples vision? The invasion to our speciality is from many other specialities, and the only reason is your money. An authentic Plastic Surgery indeed needs a skillfully demanding ability and artistry combined along with passing the most complete program that gives credit to his qualification. As an analogy: anyone can read a cooking recipe, have the right and fresh ingredients and the oven or whatever needs, and the dish can be “prepared” by thousands of people, but just a few are gifted with the good taste. Be careful, do your homework: ASPS is the most important Society of Board Certified Plastic Aesthetic and Reconstructive Plastic Surgeons. Remember: there is only one chance to perfection, and also another for a lifetime disaster. You will remember this, everyday when you look your self in a mirror. I wish, with a smile.

It is not the implant itself, but the amount of adipose tissue you have. Maybe you are skinny. If not, you have a very good solution as a graft from your own fat can be injected and partially or totally improve the problem.

You must have been earned about how important is the breast massage to avoid capsular contracture. That is not normal(pain) and you have to discard a contracture before it progresses. Sorry if I am that direct. Go with your surgeon and do the pertinent studies to discard this diagnosis.
Does my revision chin implant look normal?

Well, it looks that your Microgenia case, was for more than a chin implant, as projection is suboptimal. You may need to “cut and advance your own chin bone. Cephalometry will confirm this.
Which procedure would give me the most positive & drastic change?

Well, your own perception is too drastic : in my opinion you are a beautiful woman and a good Aesthetic Rhinoplasty would make a positive change in your face, adding an effective “added charm”, just like the “jewel in the crown”. 2 year post op scar

A scar revision is possible, even it looks it is not easy to guarantee a total improvement. The reason why it happened is just because an improper wound lips closure an an extended incision. If you ever decide to do a scar revision, of course go with a Board Certified Plastic Surgeon and you will need a laser treatment a few weeks after( Picolaser). Well, it all depends on your own natural features and anatomical needs vs real and natural looking results. For instance, some people comes and ask for eyelids surgery, when they are still young and an improvement is reached with a tail of the eyebrows lift. There are techniques for treating the lateral cantus in order to avoid or to correct an ectoprion during blepharoplasty. Maybe, what you are looking for, is what is being promoted as “Foxy eyes” that is a combination of tail of the eyebrowlift and upper cheeks bone and cheeks lift. You must be evaluated( photoss or personal consultation) to see realistic expectations and possibilities.

It all depends of the problem’s dimension ( no photographs). Melatonin stimulation, laser or even therapeutic tatoo, may be considered.

Umbilical hernia, is a frequent abdominal wall defect as common as 1:1,000 frequent in population. Usually, we do a Mayo technique repair and recurrence is less than 0.5 %. Revisional or secundary hernioplasty is mandatory for this case.

Can ruptured implant be left alone

Well, in my opinion, you shouldn’t be afraid of deflated implant removal, specially if it is that old and deflated. The implant’s surface material will always be a foreign body for your breast and there is no reason to give a way to speculations and worries about caring about your health. Procastination is not the answer. The surgical procedure is relatively easy, but the foreign body’s and your biological healing response , may not be necessarily “friendly” and the surgeon who attends your case, can find a “mass” or hard to remove residual implants surface. I attended a similar case of 18 years after procedure, and reaction many times depends of the implant’s make too.
Wound Care

Wound healing process is by steps and sometimes a minor “open wound” area, may occur. The important issue, in your case( by the photo and your comments) is that there are not sign of infection By advice is to keep the wounds perfectly clean( Iodine or similar), you can try to re adapt the wounds lips and let the tape work by 3-5 days. Anyway, there are many other options (like using Sunscreen like Isdin, scar massage and remember that final results are seen after 1 year postop. Relax, Do not break contact with your surgeon( he made a good job), and just… give a time for the healing process.

By the photograph, you have a very charm and balanced face. Clinical photography( front, 3/4, profile, basal, with hair combed /ponytail type)) may give more elements for an accurate diagnosis.
