Hi Melissa. I always try to accommodate my patients’s goals, but choosing the right implant for a patient really depends upon multiple factors including a patient’s goals, and their anatomy. Placing 800 cc implants into someone who starts out with an “A” cup is likely not possible on the first go around. I personally don’t get all the hype about prefectural implants. In my opinion, it’s like trying to rediscover the wheel. Subglafukar breast implants initially always were less painful( the muscle was not cut, or stretched from an implant)., however long term rippling and higher incidence of capsular contracture were noted in this plane. Now you have a breast with even less soft tissue coverage, and will have a higher incidence of rippling and visibility. We’ve know this for years with subglandular breast augmentation. Yes you can add fat grafting, but in my opinion it is likely going to be less aesthetic than a submuscular one

Usually when I remove implants I can sometimes perform a Lift at the same time. In some patients where they do not have a lot of breast tissue and have very large implants, it sometimes is good to wait until the tissues collapse down before performing a lift. This also is a consideration when performing fat grafting. If there is a large open space then the fat will not have any blood supply to maintain their viability, so in these cases I will also sometimes wait to put that in if they so desire.

Drain less tummy tucks arevoften performed with internal quilting futures or what are sometimes called progressive tension sutures. Some surgeons also spray fibrin glue internally as well. I’m not sure why you were so reluctant to have drains. Drains are usually removed within about a week or so for most patients. I do not think that going drainless adds any advantage Patients can develop seromas and hematomas as well. I do all the other procedures you desire as well. Good luck.

I’d like to think that I guide my patients in a way that they maintain a nice youthful look without looking what I call “overly plastic.” Many patients in their mid-40s can have very nice results with fillers and Botox alone. There are some at this age that have significant facial aging and can benefit from facial rejuvenation surgery like a facelift.
