With over 20 years of practice in Palm Beach County, Dr. Rosenthal has been consistently recognized by his peers as a Top Doc in Palm Beach Illustrated for the past 5 years. A prominent leader in plastic surgery, he currently serves as President-elect of the Florida Society of Plastic Surgeons (FSPS) and holds several leadership positions with the American Society of Plastic Surgeons (ASPS). Congratulations for staying so fit! But all the working out in the world unfortunately won't fix sagging or thinning skin. Skin loses substance over time due to genetics, environment, and sun damage. However, we have more options than ever to slow down and even reverse this process. They key is to get the skin to regenerate and line up those collagen bundles to be more youthful. This is usually accomplished through energy treatments (RF or light based) to make the skin behave again. All of those bumps are just seeing imperfections of the deeper tissues through damaged skin. RF microneedling and a process called SkinTyte are two good modalities which we use. Think of your skin as a thick blanket which used to cover things. Now that thick blanket has become a thin sheet over time. Energy based treatments can thicken the skin again as well as remove wrinkles and improve irregular pigment and sun spots.

I am sorry you have struggle with this asymmetry. It is possible to meet somewhere "in the middle" by reducing and lifting larger breast and augmenting the smaller one with fat. Reducing the areola size will also help. Insurance coverage varies. Be sure to seek out a consultation with a Board Certified Plastic Surgeon. A telehealth visit may be an option for an initial consultation! There are many fine plastic surgeons here in Florida.

Joanie- Thanks for your question. The most important thing to do is have a proper diagnosis of your cosmetic situation. There are many minimally invasive and noninvasive options that are available that weren't around even a few years ago. The first step is evaluation and assessment of your goals. You just can't compare prices and go with that. You also have to have a relationship with your surgeon and comfort level. I would generally agree with Dr. Rosato on this. And it's ok to travel for something if you really connect but take ease of access in the postoperative period is also important. This is a journey.

Many women are describing a variety of symptoms some time after implant placement, a widely varying syndrome that has been known as BII. The ASPS continues to work very hard to get to the bottom of this to help women who are suffering a wide variety of symptoms and concerns. Many of our patients feel better and have benefited from implant removal (with or without fat grafting). It is very satisfying to see them "getting back" as you said. The best next step is to get a consultation with your previous surgeon or another Board Certified Plastic Surgeon either in person or via telehealth to get a clearer picture of your concerns and struggles. Your surgeon can work with your other doctors to make sure any other problems are addressed and that implant removal is right for you.

Gold threads are simply intended to create an uncontrolled foreign body reaction (i.e. scar). It is an uncontrollable result which varies from person to person and can be a nightmare if there is a problem because retrieving those foreign bodies (the gold) is nearly impossible and can cause damage to the vital structures of the face including the nerves that control facial movement. Seek a Board Certified Plastic Surgeon for an in person or telehealth consultation on what other options may be available for you to achieve your goals.

Surgical risks on immune modulating agents are manageable. Dosage and past history matters as well. Make sure you have a board-certified plastic surgeon who can work with your rheumatologist to effectively assess the risks and benefits. There are many less invasive procedures now to address these areas for which you may be a good candidate.

I am sorry you are struggling with this but there are often good options without using implants. There are different conditions that can lead to breast asymmetry. The most important thing in plastic surgery, like any other medical specialty, is to make the correct diagnosis. Once you make the correct diagnosis, then come up with the right plan-- right for the problem and right for the patient taking their desires into account. Asymmetric reduction with fat grafting is probably the best option, but it would be important for you to see a board certified Plastic Surgeon to make sure that is the option that is right for you and your specific situation.

Asymmetrical breasts can be very troubling. Some asymmetry is very common but large asymmetries are very difficult but your difference is definately sizable. The best option in this case I have found with the information you have given is to reduce the larger breast some and augment the smaller one to meet somewhere in the middle. The augmentation on the right can often be done with fat transfer if implants aren't an option or aren't desired. Seek a consultation with a board certified plastic surgeon who is comfortable and experienced with these types of situation. There are options! And consultations can be done both in person and often with telemedicine video chat.

Inframammary Fold - Breast Reconstruction Revision

It appears the soft tissue pocket is inadequate for the implant. Also blunting of the fold is very common in an implant reconstruction. There are "tweaks" that can be performed to improve this. Also sometimes using either acellular dermal matrix (ADM) such as Alloderm or other absorbable mesh can be used to improve the result. I would caution that many insurance companies are denying these adjustments as "cosmetic" and may deny coverage. Be sure to discuss this with your Board Certified Plastic Surgeon so you don't receive an unexpected bill. We are working at a national level to combat these types of issues. The Women's Health Care Rights Act of 1998 needs to be updated to address differences in cancer care and breast reconstruction available over 20 years later since the legislation was written.

Unequivocally yes. Handling previous health struggles (and that is exactly what this is) is not necessarily a contraindication to cosmetic or aesthetic surgery. It does need to be recognized and managed. You should seek a board certified plastic surgeon who understands your history and manages it and your expectations appropriately. We assist patients in recovery from a many conditions with in-office evaluation, support, and counseling as necessary. First your surgeon will need to make sure there is not a medical or metabolic cause of this. Kybella is a good option for facial contouring to reduce volume. Buccal fat removal is also an option. This is often called a "plus/minus" operation because you are adding implant volume (plus) where you want it and removing breast volume (minus) where you don't. This can be a great operation in a breast with undesirable architecture for just a lift and give you a lot of flexibility in size. I rarely find it needs to be done as a staged procedure if done with the "vertical" technique with a superior-medial pedicle. Make sure it is being performed by a board certified Plastic Surgeon with extensive experience in this type of procedure. Also experience in difficult breast reconstruction is a benefit.

Thanks for your question. This is a difficult problem for many women. Breast reduction paid for by insurance used to be a simple equation based on amount of breast tissue removed and your body surface area (which is a calculation of height and weight). Times have changed. Most policies have weight requirements as well as others for documentation of rashes, back and neck problems, etc. not fixed by other interventions (dermatology treatments, physical therapy, weight loss, wide strapped bras and NSAIDs). This is unfortunate since even modest reductions can help women. Check your policy for individual requirements. For significant, long lasting upper pole fullness, fat grafting or an implant may be required. These portions are not usually covered by insurance. Make sure you get a pre-approval (which may be a long process) or you may get stuck with high bills if your insurance eventually denies it.

As many of the other Board Certified Plastic Surgeons here have indicated, differences in breast size are very common! This can be a simple size difference or more complicated issues may exist leading to these differences in size and/or shape. Fat grafting is a great tool for helping to treat some of these differences. Asymmetric reduction (one side more than that other) is often a good option as well. SMALL differences can be addressed with different sized implants if you want the breasts larger but I don't favor this approach in most cases. Fat grafting, asymmetric reduction/lift, and implants can all be used together. The most important thing is to find a board certified plastic surgeon who can work through all of these options with you. But significant size differences are NOT to be taken lightly and not (usually) just by a straightforward breast augmentation. The good news is that with the proper diagnosis & tools, most women's concerns can be addressed and fixed.

The most important thing is to COMMUNICATE and CONNECT with your surgeon. This is true of any procedure but especially rhinoplasty. From your question, it doesn't sound you are satisfied with either response thus far. Rhinoplasty is an incredibly complex operation where balance and function (breathing) are the most important goals. Sometimes you don't need to LIFT the tip but it is important to support it since altering the bump may unwittingly affect the tip or its appearance. Make sure you feel comfortable about the reasons the specific treatment is being suggested and, although pics can be helpful, it is much easier to fix pixels on a screen than noses! Make sure you are seeing a board certified experienced rhinoplasty surgeon and you feel you can trust. Redos are much more involved than getting it right the first time.
