To get a look into what's making waves in plastic surgery for 2025, we spoke with several ASPS Member Surgeons about the upcoming trends they foresee.

Smaller breast implants are in

Big, bold implants are taking a backseat as more patients choose smaller, natural-looking enhancements.

"Breast implant size is going down," said Praful Ramineni, MD. "Patients getting breast reductions want to be smaller sizes, sometimes a B or a small C. The era of going really large is gone."

This shift reflects a growing desire for subtlety and comfort, as patients prioritize proportions that better fit their frames. Smaller implants also come with fewer long-term risks, making them an attractive option for anyone considering breast augmentation or reduction.

A new breast implant

Women augmenting their breasts or reconstructing them after cancer now have access to a new implant from Motiva, which is designed to look and feel natural. Surgeons and patients alike are raving about the results.

"I've just started using them," said Dr. Ramineni. "Patients so far vastly prefer the Motiva implants in terms of feel and look."

These implants offer a teardrop shape that mimics the breast's natural contour, making them ideal for those looking for subtle enhancements.

"Many patients who are flat-chested want natural-looking breasts and don't want that round appearance," said Dr. Ramineni. "So, for somebody who wants a B cup, the gel will fall to the bottom of the implant and give them more of that teardrop shape."

A natural breast is slightly flatter on top and fuller on the bottom, and many patients prefer that style.

Using your body's own platelets and fat

More people are opting for beauty treatments that come from their own bodies. Some standout options that are gaining popularity for their natural approach to anti-aging are injections like platelet-rich plasma (PRP) and a newer version called platelet-rich fibrin (PRF). In addition, using your body fat to enhance your looks is becoming popular.

PRP is a concentrated solution of platelets derived from your own blood. When injected or applied topically, growth factors and proteins promote healing and tissue regeneration. It can improve thinning hair and reduce fine facial lines.

PRF is a next-generation version of PRP that slowly releases growth factors over time. It improves thinning hair and also promotes tissue regeneration when injected into the face or hands.

Fat transfer includes filling lips, restoring lost facial volume and plumping breasts with fat liposuctioned from other body parts like your abdomen or thighs.

By using your body's own healing abilities, these treatments deliver subtle, natural results. If you want to refresh your appearance without anything artificial, these options are leading the way.

Younger patients opt for facelifts

Facelifts are no longer reserved for seniors. Today, more people in their midlife are embracing this surgery as part of their anti-aging routine.

"In my practice, I see more women in their 40s and 50s opting for a facelift earlier, rather than exhausting every nonsurgical option before considering surgery as a last resort," said Ira Savetsky, MD.

Having a facelift at a relatively young age means a woman can enjoy her surgical results earlier in life. Plus, it has additional perks.

"The advantage of undergoing procedures at a younger age is that we're generally healthier, more mobile and have fewer comorbidities (medical conditions), leading to smoother recoveries and better outcomes," said Dr. Savetsky.

The "Miami thong lift" for subtle curves

Brazilian butt lifts, or BBLs, may have dominated the last decade, but 2025 is all about the Miami thong lift. This new technique focuses on smoothing, tightening and lifting the buttocks.

"The 'Miami thong lift' is a combination of a cellulite treatment and a conservative lift to the buttock without adding volume," said Kristy Hamilton, MD.

Miami thong lifts smooth cellulite bands, tighten the skin and create a perkier look – all without adding the fat and dramatic curves of a traditional BBL.

"I think the era of the overdone BBL is over," said Dr. Hamilton. "For women who want to tighten and lift everything without pumping up the volume, the Miami thong is great because the incisions are hidden in the gluteal cleft, which is between the cheeks, so you don't see a scar."

Additionally, the recovery for the Miami thong lift isn't anywhere near as intense as the BBL, which requires patients to avoid sitting for weeks.

"You can sit on it after because there's no fat transfer," said Dr. Hamilton. "So, if you like your volume, you just want it a little bit perkier, it's a great option."
