Procedure
Breast Implant RemovalMember Response:
I usually deflate the implants in the office to let patients see what they would look like with an implant. Then, they can be removed through a small incision as an awake procedure.
Procedure
Question:
I want to know if it is always necessary to have the large incisions to remove saline implants. Is it possible to deflate the implant than remove it through an incision around the areola.
Karol A. Gutowski, MD, FACS's Response:
I usually deflate the implants in the office to let patients see what they would look like with an implant. Then, they can be removed through a small incision as an awake procedure.
Full or mini TT or other options?
Tummy TuckMember Response:
Since you have loose skin above your belly button, a fully tummy tuck may be better for you. With so much weight loss, a body lift is also an option. An in person assessment is best.
Full or mini TT or other options?
Question:
46 years old and recently lost 70 pounds over 10 months. I’m 5’6, 135 lbs now., and do resistance training 3 x a week. I was hopeful my skin would tighten, but the sagging isn’t pleasant. What is the least invasive procedure that will give me results that I can live with? In the final photo, I’m extending my belly to lessen the effect and would be happy with final results that look like that.
Karol A. Gutowski, MD, FACS's Response:
Since you have loose skin above your belly button, a fully tummy tuck may be better for you. With so much weight loss, a body lift is also an option. An in person assessment is best.
Consultation
Male Breast ReductionMember Response:
I have availability for a consultation and in most cases can do gynecomastia surgery as an awake procedure with a local anesthetic.
Consultation
Question:
I'm trying to schedule a consultation for Male Breast Reduction (Gynecomastia). The recommended Board-Certified Plastic Surgeon, who is also a member of the American Society of Plastic Surgeons (ASPS), doesn't have consultation openings until May 2025. How can I find a surgeon - in my area with the same qualifications who may be available much sooner?
Karol A. Gutowski, MD, FACS's Response:
I have availability for a consultation and in most cases can do gynecomastia surgery as an awake procedure with a local anesthetic.
Age Requirement
Breast ReductionMember Response:
Age alone will not exclude her from having a breast reduction. However, a discussion with a plastic surgeon and the pros and cons of surgery is the first step. I have treated a few patients in their teens and all were happy to have it done for symptom improvement.
Age Requirement
Question:
I have questions about age requirement, my daughter is 16 years old and weights about 100lbs, she’s 5ft. she’s petite! & the issue is she suffers a lot with back pain, rashes in between her breasts & insecurities she can not control. She’s a 32DD, i have records of the chiropractor, doctor visits for rash cream & she’s been going to therapy for her mental health,im searching for the ideal surgeon
Karol A. Gutowski, MD, FACS's Response:

Open wound, wet scab?
Breast LiftMember Response:
If the dressing is sticking to the wound, apply a thin layer of antibiotic ointment or Vaseline to the wound first. Or, use a nonstick dressing like the inside of a Band-Aid.
Open wound, wet scab?
Question:
I had a mommy makeover on July 8th and I'm concerned about how this incision is healing. My plastic surgeon is on vacation and I have been messaging her through text. She suggested that I change a dry dressing three times a day but every time I go to change the dressing it is sticking to the area. This morning I used sterile gauze and saline solution to blot the area. Any suggestions? Thank you
Karol A. Gutowski, MD, FACS's Response:
If the dressing is sticking to the wound, apply a thin layer of antibiotic ointment or Vaseline to the wound first. Or, use a nonstick dressing like the inside of a Band-Aid.
Vermilion Border Scar
Cleft Lip and Palate RepairMember Response:
Based on the picture, it healed well. If it bothers him when he is older and can make a decision on treatment, it can be revised.
Vermilion Border Scar
Question:
Hello, my 1 year old was injured about 3.5 months ago and this is the scar (top left side) that has formed on his lip, crossing his vermilion border. Does anyone have experience with a similar scar in young patients and how did it eventually end up healing? I am worried it will show this wide gap in the border forever.
Karol A. Gutowski, MD, FACS's Response:
Based on the picture, it healed well. If it bothers him when he is older and can make a decision on treatment, it can be revised.
Best Facial Procedures
RhinoplastyMember Response:
Without knowing your goals, I wouldn't trust anyone to give you advice on what to do. "Crowd sourcing" recommendations for surgery is not a substitute for a comprehensive evaluation and individualized treatment plan. Spend some time discussing this with a board certified plastic surgeon.
Best Facial Procedures
Question:
Hello, I would like to the most minimal amount of work with the biggest impact. I'm looking to do simple changes to both keep a natural look and for financial reasons. I've had advice to do rhinoplasty and perhaps a procedure to decrease jowels. However, others have suggested lip augmentation and fillers to achieve the best results. Not really sure what I should prioritize to achieve my goals.
Karol A. Gutowski, MD, FACS's Response:
Without knowing your goals, I wouldn't trust anyone to give you advice on what to do. "Crowd sourcing" recommendations for surgery is not a substitute for a comprehensive evaluation and individualized treatment plan. Spend some time discussing this with a board certified plastic surgeon.
What can I do
Breast AugmentationMember Response:
You need to see a local plastic surgeon as soon as possible.
What can I do
Question:
I am about 16 days out from getting my breast augmentation with implants and a lift. Recently I had a lot of drainage and now my incisions have started to open. I have contacted my surgeon but due to him being in a different country it takes awhile for him to respond. Is this normal or is there anything I can do to help it close? I change my gauze 3-4 times a day and clean everything.
Karol A. Gutowski, MD, FACS's Response:
You need to see a local plastic surgeon as soon as possible.

Buried Penis next steps
Question:
Due to the limitations of 400 characters, this will be greatly reduced in length. I'm an adult male with an infant botched circumcision, I've lost 300 lbs (now 5'11 178) and undergone panniculectomy and two monsplasty procedures. The penis remains fully buried, and I can insert three inches of my finger into the hole that exists. What can be done for me? I need penile reconstruction asap.

Is there help for Lipedema
Thigh LiftMember Response:
I've had a few patients with lipedema from Colorado come to see me in Chicago.
Is there help for Lipedema
Question:
am in CO and I have pretty severe Lipedema in my legs to the point I am having trouble walking. I am a SPED Paraeducator and I have 4 kids so I am on my feet a lot. I desperately need help. Is there any doctor willing to help me, I am truly scared about losing the use of my legs?
Karol A. Gutowski, MD, FACS's Response:
I've had a few patients with lipedema from Colorado come to see me in Chicago.
MOHS repair on nose
Skin Cancer RemovalMember Response:
Maybe a forehead flap or bilobed flap.
MOHS repair on nose
Question:
I'm wondering what the most likely method of repair would be after this MOHS surgery? As you can see the surgery was extensive and I'm wondering what the best course of action would be.
Karol A. Gutowski, MD, FACS's Response:
Maybe a forehead flap or bilobed flap.
Explant over muscle uneven natural breasts
Breast ReconstructionMember Response:
In most cases, saline implants can be deflated as an office procedure (or removed) and then you can wait 3 months to see how they will look. After that, fat grafting, breast lifts, or other treatments can be considered.
Explant over muscle uneven natural breasts
Question:
had breast implants for almost 24 years. Over the muscle, saline. Before One breast a cone shape size A with a puffy nipple and the second a droopy but 'normal' barely B with a large areola. They never looked right. Now after pregnancy, and years going by they look and feel horrible on both sides. I want them out, not replaced. What will the best procedure be after explant as symmetrical as can
Karol A. Gutowski, MD, FACS's Response:
In most cases, saline implants can be deflated as an office procedure (or removed) and then you can wait 3 months to see how they will look. After that, fat grafting, breast lifts, or other treatments can be considered.
Can ruptured implant be left alone
Breast Implant RevisionMember Response:
Unless you are having a problem with them, there is no medical reason to remove a ruptured SALINE implant. Its no different than a non-ruptured implants, its just empty with a small hole in it. The is no evidence that it will lead to capsular contracture. For patient who want them removed, I usually do it awake with just a local anesthetic.
Can ruptured implant be left alone
Question:
I have 30 year old saline implants & the right implant deflated. They are small implants so it’s not really noticeable. I am terrified of surgery & wondered is it medically necessary to have it? What risk is there in leaving as is? I should note I have an occasional jarring pain in the right side (under breast toward the armpit) over the last few years which my obgyn doesn’t think is a concern
Karol A. Gutowski, MD, FACS's Response:

Breast explant and lift
Breast LiftMember Response:
It is best that you contact your surgeon today about this and share your concerns to get appropriate care.
Breast explant and lift
Question:
I was told not to remove tape so I can't see my incisions well. I have 3 concerns 1. The color of my nipples (the first pic is 5 days post op) has gotten drastically darker (2nd & 3rd pic, 10 dpo) 2 . Concerned with the extra skin visible on the vertical incision that seems to be bugling and it seems the incision is not straight 3. Concave nipples post surgery, nipples were not concave preop

It is best that you contact your surgeon today about this and share your concerns to get appropriate care.
Necrotic tummy tuck
Tummy TuckMember Response:
Not all wounds are pain free, some pain early on is common. Discuss this with your plastic surgeon.
Necrotic tummy tuck
Question:
I had a TT&ended up with necrosis/open wound. It is healing slowly&I am putting gaze soaked in water& cleaning it in the shower as instructed.I am having pain in any position besides laying down. The bottom region from the incision&below burns. The corners have tunnels, discharge&hurt.I’m told I shouldn’t have pain&should be able to go back to work.I don’t foresee being able to.Is pain atypical&work e

Explant procedure without capsule removal
Breast Implant RemovalMember Response:
Since you are not having any problems, unless they are ruptured, there is no need to remove the capsule. However, it is best to have a discussion with an experienced plastic surgeon. If you have textured Allergan implants, it may be worth discussing capsule removal.
Explant procedure without capsule removal
Question:
I want to remove my silicone implants after having them for 13 years. Never had any problems with them doesn’t feel like they rupture everything fine. I want an explant procedure without capsule removal. Is it harmful/dangerous if I leave capsule?
Karol A. Gutowski, MD, FACS's Response:
Since you are not having any problems, unless they are ruptured, there is no need to remove the capsule. However, it is best to have a discussion with an experienced plastic surgeon. If you have textured Allergan implants, it may be worth discussing capsule removal.
Where to go for Congenital Symmastia?
Breast ReconstructionMember Response:
I am in Chicago and have experience correcting congenital symmastia. I may be able to do it without hospital fees.
Where to go for Congenital Symmastia?
Question:
I have congenital symmastia, and was wondering if there would be surgeons and/or university residents who would be interested in taking on my case for a reduced fee/trying to finagle with insurance? I know it’s rare, so what an opportunity for some up and coming surgeons! Almost like participating in new research. Located in Michigan, USA (can’t find surgeons in the area, period).
Karol A. Gutowski, MD, FACS's Response:
I am in Chicago and have experience correcting congenital symmastia. I may be able to do it without hospital fees.
Realistic lift goals?
Breast LiftMember Response:
From what you describe, your goals are appropriate, but an in-person assessment is the only way to find out. Fortunately, Dallas is home to many great plastic surgeons who can help you.
Realistic lift goals?
Question:
I'm 36 & 5'6. When I was 19 I gained 50 lbs, got to 175 lbs & wore a size 32J bra. I then lost 50 lbs in my late 20s due to hormonal changes (weight has has stayed at 125 for 7 years). I now wear a 30E bra size. I have healthy breast tissue & not much fat. I'm considering a lift (no implants or reduction) to improve appearance & clothing fit and reduce strain on my back. Are my goals realistic?
Karol A. Gutowski, MD, FACS's Response:
From what you describe, your goals are appropriate, but an in-person assessment is the only way to find out. Fortunately, Dallas is home to many great plastic surgeons who can help you.
Regarding consent
FaceliftMember Response:
This may be better to discuss with the doctor who uses the form as its hard to comment washout knowing the full context of the situation.
Regarding consent
Question:
ASpS offers a consent form labeled Shorter version. When and why would a physician offer this consent to his or her patient? Can you explain the disclose at the very bottom of this consent?
Karol A. Gutowski, MD, FACS's Response:
This may be better to discuss with the doctor who uses the form as its hard to comment washout knowing the full context of the situation.
tummy tuck and lift
Body ContouringMember Response:
In most cases, a lower body lift ( a continuation of the tummy tuck) is done at the same time. Perhaps there is a misunderstanding in what you are having done?
tummy tuck and lift
Question:
Does separating lower body lift from TT give better results? I was told to have extended tummy tuck and then have lower body lift in a second surgery. It was explained there is vertical pull down for a TT and then opposite side for hip/butt/thigh lift and results in stomach not being as flat. Its hard to navigate when consults are so different.
Karol A. Gutowski, MD, FACS's Response:
In most cases, a lower body lift ( a continuation of the tummy tuck) is done at the same time. Perhaps there is a misunderstanding in what you are having done?
Complications after tummy tuck
Tummy TuckMember Response:
In most cases, a "dog ear" correction and liposuction of the flanks can be done awake with a local anesthetic which makes serious complications extremely rare.
Complications after tummy tuck
Question:
I had a tummy tuck in 4/2023. I developed a small blood clot in my lungs. I'm still on blood thinners for this and will be for the next few months. On my 6 week check up my surgeon asked if I was interested in dog ear revision with lipo to the flanks. I said no to the lipo because I'm scared to go under again. But are there any serious complications of dog ear removal if it's a minor procedure?
Karol A. Gutowski, MD, FACS's Response:
In most cases, a "dog ear" correction and liposuction of the flanks can be done awake with a local anesthetic which makes serious complications extremely rare.
Unsuccessful Search
Breast Implant RemovalMember Response:
This is something that I offer but of course it depends on how much fat you have available to transfer and if a crescent mastopexy is appropriate for you (it usually isn't after implant removal). Feel free to contact me,
Unsuccessful Search
Question:
I've been searching the internet for months looking for a surgeon who can remove my breast implants, perform a Crescent Mastopexy, and fat grafting to my breasts with minimal scarring. My breasts are beginning to sag, since I stopped my breastfeeding journey. I have also become increasingly paranoid about BII. Can anyone recommend a skilled surgeon or have experience with this procedure? Thank you
Karol A. Gutowski, MD, FACS's Response:

Malignant hyperthermia
Tummy TuckMember Response:
In addition to Dr Culbertson's advice, some surgeons do tummy tucks without the gas that can cause. I've done it with TIVA (Total Intravenous Anesthesia) + tumescent anesthesia which doesn't require any gas and the patient is breathing on their own. Happy to discuss further.

Question:
I am very interested in having a tummy tuck but I do have MH. I am having difficulty finding a surgeon who will take me on. I live in Fauquier County but am willing to travel for the right surgeon. Any suggestions on how to find one?

Lipadema Thigh Lift
Question:
I am looking for a surgeon that accepts Tricare insurance and that specializes in lipadema. I need a thigh lift and lipo to my calves for lipadema. I have already had liposuction on my thighs once before, so they have been deflated and they look terribly deformed. My thigh skin hangs below my knees on the side of my legs. Wouldn't that be included in the thigh lift?

SAFELipo vs SmartLipo re: anesthesia
LiposuctionMember Response:
I agree with the other surgeons. Its the surgeon not the machine that delivers results. If the same areas are treated and the same amount is removed, then the recovery is the same regardless of technique. I would even add that in most cases, anyone who claims that one technique is much better than another without showing well done studies to support the claims is not being honest. In certain cases, there may be an advantage to a specific technique. Don't be fooled into thinking that "SAFElipo" is safer or better than any other lipo (no evidence to support that) or that "SmartLipo" is any "smarter" (it isn't). All techniques can be done awake or with sedation or full anesthesia. Want to learn more, watch this video: https://www.youtube.com/watch?v=05IULCdxNyc
SAFELipo vs SmartLipo re: anesthesia
Question:
I’m a 45 yr old female in great physical health with a BMI of 23 trying to decide between SAFELipo (PAL) and SmartLipo. Cost difference is $28k vs $15k, recovery difference is 6-8 weeks vs 1-2 weeks & anesthesia difference is general vs local. I’d prefer not to go under general, but want the best result. Can anyone tell me why the huge differences? Are there reasons to choose one over the other?

Operative Report Question
RhinoplastyMember Response:
It is possible that the surgeon made a dictation error. If a graft was used, there may be a record of the graft in the medical record to confirm that it was placed.
Operative Report Question
Question:
Can a Operative report be amended a month after surgery? (original report said “harvested septal cartilage was used” the new operative report says “MTF rib cartilage used” instead this got amended by the surgeon after I asked if he ended up using the rib cadaver I paid for upfront. It just seems strange he changed that word the day I asked the question.
Karol A. Gutowski, MD, FACS's Response:
It is possible that the surgeon made a dictation error. If a graft was used, there may be a record of the graft in the medical record to confirm that it was placed.
Cheek/Chin implants
Cheek AugmentationMember Response:
An in person assessment is the best way to find out but based on the pictures, some check enhancement may help. You can start with an injectable filler before having implants.
Cheek/Chin implants
Question:
I dislike the way that I look and would like to improve my appearance through surgery. I have a hard time pinning down exactly what would improve my appearance. I think I would benefit from cheek implants and chin or jaw implants. I’ve seen a surgeon and am not a good candidate for double chin lipo (it’s excess skin, no fat present). Do I look like I would benefit from these?
Karol A. Gutowski, MD, FACS's Response:
An in person assessment is the best way to find out but based on the pictures, some check enhancement may help. You can start with an injectable filler before having implants.

Is Liposuction dangerous with a lower body lift?
Question:
Im having a lower body lift with Brazilian butt lift and breast lift/augmentation. My surgeon stated he will not lipo my waist with the lift because it will risk killing the healing skin... however it seems like they usually come hand in hand. Will I get the hourglass waist without lipo? Should I add lipo before or after the surgeries? Are my expectations too high?
Karol A. Gutowski, MD, FACS's Response:
Based on the information you provided, liposuction of your midsection would be appropriate during the body lift. Consider discussing this again with your surgeon or getting another in-person opinion.

Can I get a breast reduction if my breasts keep growing?

This is a good question to discuss with a plastic surgeon and determine why your breasts are growing. An in person evaluation is the best way to start. If your symptoms are severe, it may be appropriate to do a reduction now.

Determined to get a lift for the back rolls that have always been a problem and more so after gaining 40 pounds and losing it. Saline implant deflation and wait several months?
Breast Implant RevisionMember Response:
Very reasonable. I would do the same.
Saline implant deflation and wait several months?
Question:
Hi, I've had saline implants for 10+ years and it's time to redo them (too big, uneven height). My surgeon suggests that we deflate and drain them now, let the skin heal/shrink back if possible then do the new breast augmentation in 3-4 months. The goal being that perhaps I'll need less of a breast lift and smaller implants placed if we give it more time to heal. Is this a realistic plan?
Karol A. Gutowski, MD, FACS's Response:
Very reasonable. I would do the same.

Insurance Red Flag
Male Breast ReductionMember Response:
There is nothing wrong with accepting insurance. If a doctor accepts an insurance plan, then they have to accept it for all procedures. However, most insurance plans don't cover it so just because a surgeon accepts insurance, it doesn't mean that insurance will pay for it.
Insurance Red Flag
Question:
Is it a red flag for a plastic surgeon at a UAB (Birmingham, AL) clinic to accept insurance for gynecomastia surgery? Does it mean that the surgeon might be desperate for patients, etc.? Dr. Lebowitz said this to me and I saw it on Dr. Carini's website. Why do you think this is the case?
Karol A. Gutowski, MD, FACS's Response:
There is nothing wrong with accepting insurance. If a doctor accepts an insurance plan, then they have to accept it for all procedures. However, most insurance plans don't cover it so just because a surgeon accepts insurance, it doesn't mean that insurance will pay for it.
Incision
Tummy TuckMember Response:
Most surgeons have a revision policy that you would have seen before the procedure. As Dr Johnson pointed out, many surgeons will not charge their fee to revise this, but there are other expenses that surgeons can't absorb.
Incision
Question:
I had a tummy tuck a year ago. My incision opened during recovery and did not heal properly. My doctor said he needed to cut it and restitch the incision. Today I went to my doctor and he said I can have the scar repaired and he wouldn’t charge his fee but I would be responsible for surgery center, $1200. Should I have to pay to have this incision repaired or should it be dr responsibility?
Karol A. Gutowski, MD, FACS's Response:
Most surgeons have a revision policy that you would have seen before the procedure. As Dr Johnson pointed out, many surgeons will not charge their fee to revise this, but there are other expenses that surgeons can't absorb.

Scar in chin/neck
Question:
I have a scar on my neck/ chin area from when I was 5 years old and a dog bite. I have scars near my eye, nose, and side chin but these do not bother me. The surgeon did a wonderful job, but on my lower chin because the placement and how this skin does not drop with the rest of my face it makes me look like I have a double chin at any lower angle. Is there a way to reduce this scar?
Karol A. Gutowski, MD, FACS's Response:
It is possible that the scar could be improved but you need an in-person evaluation for a proper assessment.

Fat and unshapely legs
Question:
I’m not sure how to categorize this. So at my highest weight I was 315 pounds and at my lowest adult weight I was 165. I’m currently at about 200 but back on a weight loss regime. My legs have virtually never changed whether I was at 300 160 or even now. Is this something plastic surgery can fix, what is it called and what should I look for in a surgeon. Do I need to be closer to my goal weight?
Karol A. Gutowski, MD, FACS's Response:
You may have lipedema.

Surgery to eliminate “Doggy Ears” after my Tummy Tuck
Question:
Hello Im a 33 year old male. 11 years ago I had an abdominoplasty (Tummy Tuck) to remove excess skin (from being overweight). My stomach is still tight but sadly after surgery I was left with these “doggy ears” (Im told its called) folds of skin on both sides of my hips that protrude. Is there a procedure to remove them? I also spent 11k on the tummy tuck & am hoping this would be much cheaper?

Chest tubes
Question:
My daughter had a surgery when she was 5 years old that required chest tubes. Now at 15, the areas where the tubes were is indented, almost as much as say a bellybutton. At 15, this bothers her, I was wondering if this can be corrected and if so the risks. Thanks

Yes, these can be fixed. I typically fix them with just a local anesthetic and it can be done awake with minimal risk.

Permanent Suture Removal
Question:
I had a TT 5 years ago to repair hernias and severe diastasis recti following 3 pregnancies (one with twins). The results are good and I have restored strength to my core. Over the past 2 years the permanent suture knots have started protruding below my belly button. I’m thin so you can see them and it’s also painful! How can this be corrected? Will removing them weaken my core again? Thanks!
Karol A. Gutowski, MD, FACS's Response:
These sutures can be removed without compromising your repair. At this point, the healing is done. I've done this for a few patients with just a local anesthetic.

What would be the minimum age required to have a chin augmentation surgery?
Chin SurgeryMember Response:
There is no minimum age for a chin implant but the first thing to do is to find out if it is just your chin, or more of your facial bone structure that is out of proportion. This requires an in person evaluation to see if your jaw, teeth, and other facial structures developed properly. Of course, you need to have a discussion with your parents about how you feel. In some cases, nose and chin procedures can be done at age 15 or 16 with parental approval.

How long should I wait
Breast AugmentationMember Response:
There is no rule on how long to wait but it may be better to wait 3 to 6 months to allow your body to recover fully. Also, that would be a good time to address and touch-ups on the tummy tuck (if needed). While each surgeon may have a different view on this, in most cases, it is safe to do a breast augmentation and arm lift at the same time. Learn more at DrGutowski.com

FINDINGS: 100ccs of hematoma and “punctuate bleeders on my cheat wall. I've waited a year. I'm hideous. Would you pay someone $7500 to do this to you https://photos.app.goo.gl/wbDYaVEyWfF5ucJT9
Karol A. Gutowski, MD, FACS's Response:
Without knowing more and seeing at least your before and after pictures, it is hard to give an opinion. Start by seeing your original surgeon or another surgeon in your area. If you feel comfortable sharing your pictures, others may be able to give you an opinion.

Its hard to say without an in person evaluation of this problem, or at least seeing a picture. If you are concerned, you may want to get a second opinion.
Inverted Breast Nipple
Question:
Hello! I had breast reduction surgery with liposuction in May 2019. After first check up, it was obvious that the nipple on my left breast collapsed. After two weeks, my lady surgeon tried to recover the nipple in June 2019, procedure called debridement. This hasn't worked. I am still with inverted nipple, plus complications. Can debridement recover the inverted nipple? Thank you.
Karol A. Gutowski, MD, FACS's Response:
Its hard to say without an in person evaluation of this problem, or at least seeing a picture. If you are concerned, you may want to get a second opinion.
Is this true about surgical liposuction?
LiposuctionMember Response:
This is absolutely not true. The body has no way of knowing the number of fat cells it has. Liposuction does work - otherwise it would not have been one of the most popular plastic surgery procedures for the past 20 years.
Is this true about surgical liposuction?
Question:
A salon selling Strawberry Laser told me the following: Surgery completely removes the fat cells. When you do this, the body knows how many fat cells you’re supposed to have and simply replaces them and not necessarily in the same area. Meaning you will end up with random fat pockets. With our form of lipo, the fat cells are emptied but still remain there so the body doesn’t try to replace them.
Karol A. Gutowski, MD, FACS's Response:
This is absolutely not true. The body has no way of knowing the number of fat cells it has. Liposuction does work - otherwise it would not have been one of the most popular plastic surgery procedures for the past 20 years.

Cosmetic Face Surgery ?

Depending on your age, it may be better to wait until you are older. However, neck liposuction and Kybella injections may be an option. An in person evaluation is the best way to learn more. See examples here: https://drgutowski.com/Before-After-Photos/Face/Neck-Liposuction

If by making the "vagina less fat" you mean making the pubic area thinner, then it may be possible with liposuction. Most ASPS plastic surgeons can do this.

It really depends on you and your surgeon. They can be done at the same time or a week or months apart. Without knowing more about your case, its hard to give specific advice. Best to discuss with your surgeon and make a plan together.
Arm Lift AND Tummy Tuck
Question:
After losing over 100# on my own, my insurance is paying for an arm lift and a tummy tuck. My plastic surgeon wants to do them one week apart. Is that enough time in between? Also, which surgery is better to have first?
Karol A. Gutowski, MD, FACS's Response:

Capsule contracture
Breast AugmentationMember Response:
Treatment of capsular contracture depends on how far along the contracture is (Grade 2, 3, or 4). Ask your surgeon if montelukast would help. Depending on which implant company warranty you have, some costs may be covered. Ultrasound treatment may help also.
Capsule contracture
Question:
I just had my implants done 8.5 months ago. My left implant has a capsule they told me to take vitamin e and massage. I noticed this first 3 months after surgery, it’s gotten a lot worse since then. I don’t have to pay for this to be fixed do I? Should be covered in the first year isn’t that right?
Karol A. Gutowski, MD, FACS's Response:
Treatment of capsular contracture depends on how far along the contracture is (Grade 2, 3, or 4). Ask your surgeon if montelukast would help. Depending on which implant company warranty you have, some costs may be covered. Ultrasound treatment may help also.
Aspen Technique as alternative to surgery
Breast Implant RevisionMember Response:
Aspen ultrasound may help but despite more than 10 years of making claims, there is no well designed independent study to confirm this. So while it seems to work in some cases, more advanced cases of contracture, are less likely to respond. Ask your surgeon if montelukast would be appropriate.
Aspen Technique as alternative to surgery
Question:
Do plastic surgeons recommend the Aspen Technique as an alternative to capsular contraction corrective surgery and are there any studies to show results. I cannot find any surgeons in the area that offer such technique.
Karol A. Gutowski, MD, FACS's Response:
Aspen ultrasound may help but despite more than 10 years of making claims, there is no well designed independent study to confirm this. So while it seems to work in some cases, more advanced cases of contracture, are less likely to respond. Ask your surgeon if montelukast would be appropriate.

Fat Transfer + Breast Lift
Question:
I'm interested in a fat transfer to my breasts. I have 2 questions about this: 1) can fat be taken from multiple body parts for this procedure? (e.g. thighs, knees, and arms) 2) I've read breast fat transfers increase cup size by up to 1, but could this be increased by multiple fat transfer + breast lift procedures to keep adding volume while maintaining shape?

Question:
Hello, I was wondering how I could potentially decrease the size of my calves. My weight has fluctuated the last few years but even at my lowest weight, my calves are still fatty and my shins have pockets of fat in different areas. Thick and round nose
RhinoplastyMember Response:
There are limitations to what surgery can do and thick skin makes it hard to have a thin nose. If a few surgeons advised you against more surgery, then you should accept what you have. More surgery may cause other problems.
Thick and round nose
Question:
My skin is very thick. I had a nose job with no results. I had some plastic surgeons tell me that my skin is too thick and I was not a candidate. I want my nose to be thin. I really do not want to spend more money if the outcome is not acceptable. To go deeper in this...the surgeon killed himself a month later (I do not know why) No one wants to help me. The staff just closed the office and left

Second liposuction
LiposuctionMember Response:
Same opinion as Dr Wallach and Dr Dowbak. My experience with repeat liposuction in the same area as Laser Lipo (Smart Lipo) is that there is more scar tissue and it is harder to get good results. I tend to use Power Lipo in such cases but there is no evidence that one technique is better than another.

Question:
Hi, I'm wondering if it's possible to get a breast reduction if I have 32 B breasts. Ideally I'd like to be an AA. I've heard that it's not always recommended because it can leave you with no shape, but I have a fairly petite frame (5'3, 108 lbs) so I don't know if that would be different or not. Single mother of three and realize implants are not meant for me. Both brea
Karol A. Gutowski, MD, FACS's Response:
In most cases, implants can be removed in the office with local anesthetic for minimal cost. Ask your plastic surgeon if that is an option. For my patients this is a fast procedure with minimal recovery.
Scars
Breast AugmentationMember Response:
In most cases, your implants can be replaced through the same incision. However, each patient is different so there may be a reason why the surgeon can't do it. Consider a 2nd opinion.

Question:
I have to get my saline implants redone, as I have one that’s deflated. When I had them done I also had a lift and the dr went through my nipple so I have no scars. I want them replaced, my dr said they couldn’t go through the nipple 2nd time and is have scars even to just take them out. Is it possible to get them replaced and avoid the scars?

Removal/replace?
Breast AugmentationMember Response:
Based on the information you provided, you seem to have a reasonable request to remove them. Feel free to contact me for a consultation if your surgeon will not address this issue.
Removal/replace?
Question:
Five months ago I learned that one of my 32-year old implants is ruptured and calcified. I’ve been told numerous times by my doc that I have very dense breast but had been instructed to have annual mammograms (even upon my concern for rupture). At this time all I want is a reputable board-certified surgeon that would at least remove them. He’s Sending me for another Mam to see if worse. Should I?

Body ContouringMember Response:
This sounds like a reasonable request but it is hard to give an estimate without seeing a patient in person. If you wish, I have an office in Oakbrook where you could get a proper evaluation and quote for a procedure. You may learn more at https://drgutowski.com/

Question:
I’d like to get my arms and thighs tucked. I had a gastric bypass several years ago and this is never going away. Also I see my genetic turkey jowl starting. And my augmented breast’s need to be replaced. Can I get an estimated cost for all?

A Day In The Life Of A Plastic Surgeon
Question:
I am currently a freshman in college and I have to write an essay on the day to day life of a plastic surgeon including both cosmetic and reconstructive. The question I have is very simple. What are the tasks of a plastic surgeon? What is your daily routine for your career? Lastly, what is underrated when it comes to your profession? (ex paperwork) Thank you so much!

Excision
Tattoo RemovalMember Response:
I do tattoo excision for selected cases but not all tattoos are in an area that excision is appropriate. I'm nearby in Chicago and can see you in Oakbrook.
Excision
Question:
I am having a hard time finding a dr. who will do a tattoo excision. Is that not something that drs. are willing to do? The tattoo looks awful and I am more interested in a quick removal than the year it will take to remove it via laser.
Karol A. Gutowski, MD, FACS's Response:

Potential malpractice
Breast AugmentationMember Response:
Since there are 100's of implant styles and sizes, most surgeons order the selected implants specifically for each patient. They may stock a few common shapes and sizes but no office or hospital can stock all of the possible combinations.

Question:
Hi! I consulted with a plastic surgeon & agreed on either 350 cc or 400 cc with the gummy bear type silicone- which the doctor recommended. I decided to do it & paid the full amount upfront. After asking questions to the office, I found out they didn’t have the size nor the implants I was assured I’d get. They are trying to keep the down payment. This is illegal, right? I never signed the papers

Is 45 a good age for my first procedure?
FaceliftMember Response:
45 may be an OK time for a face lift but for many patients, neck lipo with nonsurgical skin tightening may be a better option with lower cost and less downtime (and can be done awake). An in person evaluation is the best way to find out. The eye lids can be assessed also.
Is 45 a good age for my first procedure?
Question:
I’m a 45 yo female who was recently asked if I’ve ever considered lipo for my neck. I thought I was just fat but also age. I am thinking if a lower lift including a neck lift with lower/upper bleph would be an option that would offer results without a overdone appearance.

Question:
I've been contemplating breast reduction for awhile now because my breasts are causing me so much back and shoulder pain. Everything hurts and is sore for days after any exercise. Its uncomfortable to even do it. I'm 5'2" and currently wear a 40H. I just got into that size and the bras are already starting to get too small. It's like they wont stop growing. Please help. I'm at my wits end.

Sketchy Dr
LiposuctionMember Response:
While any doctor can do lipo in their office, it does not mean that they are properly trained or qualified to do it. If someone is offering a "promotion" because they are "just starting to do it" then that should raise a few red flags (no experience, not properly trained). Find a Board Certified Plastic Surgeon near you on this site: https://find.plasticsurgery.org/

Question:
How do I check to see if a Dr. is certified to do a procedure? Are there any circumstances when a non certified plastic surgery can do lipo? This office is promoting general surgeon by offering specials because he's just starting to do lipo but I wanna check him out first. Can any Dr do lipo?

May be better to go see a plastic surgeon directly for a consultation. In many cases, a breast reduction is appropriate at your age. Find a plastic surgeon here: https://find.plasticsurgery.org
19 and Desperate
Question:
I have asked my primary care doctor about breast reduction when I was in high school. He said it would be smarter to wait as I hit puberty later than average so I waited. My back, neck, and shoulder pain have only gotten much worse and I don’t like the wait I have gained because it is hard to excercise and painful. I haven’t owned a bathing suit in over two years and I just want a normal life.

It sounds like you are a good candidate for liposuction, however, an in person evaluation is best.
Leg area
Question:
I am looking for the possibility of liposuction around the knee and upper leg area. I am in good shape but have a genetic feature of fat deposits around the knee and upper part of my leg. Also looking possible liposuction on the back part of my arms. I’m around 5’6” and weigh 130 pounds. I work out regularly. These areas of fat deposits are definitely genetic. Thanks!

Dont know what to do!
Breast AugmentationMember Response:
The best way to find out your options and pick the one best for you is with an in person evaluation. I lift and reduction can be done together, but sometimes a small implant can help give more fullness. The technique depends on how much loose skin and breast tissue needs to be removed.
Dont know what to do!
Question:
Im 33 healthy-Size 34-36dd. Lost/gained 40+ lbs several times. Had sag but no extra skin. Now-can almost fit in 34c but w/ "overflow" & ALOT of extra skin-1.5-2 inches +/-. Read about both lift &reduction. Cost shouldn't be a main focus-but it's a factor. I want them lifted but still at a d "ish"...mostly care about lifting. Which is less expensive and with least amount of scarring?

I don’t Know whinny surgeon was 28 yrs ago when I had original breast reduction. I was unable to breast-feed and had a consultation 2 yrs ago but felt that the surgeon was not thorough re: my previous scars and issues. My scars look like an upside down T and my right breast is perfect with the exception of the loss of top volume & size. My left, there is a pucker under it and nipple is weird

Question:
Hi... I had a breast reduction at age 18 in 1990. I’ve been happy until recently in my mid 40’s. I want a reduction with a possible small implant to give me some fullness up top. Overall, my breasts aren’t bad... they’re just too big! 2 baby’s and gravity... HELP!

Yes, a tummy tuck can still be done but with some modification. Either a lipoabdominoplasty or a reverse abdominoplasty are possible options. An in person evaluation is the best way to find out.

Question:
I had ER surgery in 2003 and the Dr. made his incision on upper right side. I was told that because of the incision, I wouldn’t be a candidate for a tummy tuck, is this true? What r my options?
